Create Lasting Connections with Your Patients and Their Families
Working in a Home Health Care Job on Our Team is a Rewarding Career.
At Residential, we pride ourselves on creating lasting connections between our clinicians, our patients, and their families. As a home healthcare clinician, you’ll get more time with your patient, build community, and get to see the outcomes of your nursing. It’s a benefit of working with individuals in their homes. It’s something that sets us apart and our patients and families appreciate the personal touch that we bring to each and every interaction.
Working with our patient’s primary physicians, our focus is on keeping patients at home out of the hospital or nursing facility through personalized care coordination and education. We’ve listed a few home health care jobs below that make up our team and a short description of what you can expect when working with patients in their homes.
- Nurse Case Managers
- Physical Therapists
Patients receiving physical therapy may have Parkinson’s, joint replacement, CHF or COPD, or general weakness contributing to unsteadiness, fear, and loss of freedom in activities. Physical Therapists are here to help patients build strength and balance so they can become safer and more self-sufficient at home.
- Occupational Therapists
Recognizing that the best place to get better is at home, our therapists teach patients (and the patients’ caregivers) personalized adaptations for performing their daily activities more safely and independently, moving with strength and balance, environmental adjustments, and assistive devices.
- Speech-Language Pathologists
Assisting patients with communication, motor speech disorders, swallowing difficulty, loss of language ability, and cognitive and memory conditions. They provide in-home assessments and interventions for conditions such as Parkinson’s disease, stroke, ALS, head injury, dementia, COPD, and CHF.
- Journey Counselors
Working with patients and loved ones to uncover their desires and goals for their care, in simple terms and with a special emphasis on quality of life. Through listening, empathy, and in-depth knowledge of home care services, these specialists help ensure that patients are getting the right care at the right time for their unique situation.
- Home Health Aide – CNA
A Home Health Aide (CNAs) works with our nurses to provide exceptional in-home care while empowering patients to take control of their recovery. They leverage advanced technology to case manage and streamline care coordination between the patient and the interdisciplinary team.
